Help with your care assessment
To be eligible for Government funded Home Care or Residential Care you will need to be assessed by an Aged Care Assessment Team (ACAT). This process will match your needs to the packages and care available.

Help with establishing your eligibility
Assessments for Government Home Care and Residential Aged Care are directed through the My Aged Care gateway at myagedcare.gov.au or by calling 1800 200 422. It is important that you nominate Abel Tasman as your preferred supplier during this process to ensure that the referral is directed to our team.
