We provide government-funded Support at Home Packages and private services based on the Person-Centered Care model, allowing you to have complete control over the care you receive and the professionals deliver it. We offer fully funded clinical care such as basic wound dressing, medication management and referrals to specialised allied health such as podiatry, dieticians,
GP and geriatricians. Whether you need help with daily tasks like showering and personal care, meal preparation, cleaning or transport*, our dedicated team is here to support you.

From basic assistance to specialist care, Abel Tasman’s Support at Home services are designed to meet your evolving needs and encourage your independence and participation in your own life choices. You will have a dedicated Care Partner to assist you to make informed choices regarding your package and how you choose to utilise your funding. Our services are delivered with compassion and professionalism. Client categories
There are three client categories for commencing in Support at Home: Grandfathered,  Hybrid, and  New—which were established to manage the transition and determine the funding rules and client contribution frameworks that will apply to individuals.
The detailed information is based on the new Support at Home program being introduced in Australian aged care to replace the current Home Care Package (HCP) system, with the official start date being November 1, 2025.
